VIGNETTE
In her first meeting with the certified health coach at her local health club, 36-year-old Sarita expresses frustration with her constant battles with weight loss. She has tried numerous fad diets over the years, most recently "The 17 Day Diet", which she tried with some friends after reading about it on Facebook. She has never really exercised and has come to the realization that she needs to adopt a more realistic and comprehensive approach to addressing her weight and improving her health.
After 17 days of adhering to a 1,200-calorie daily diet, Sarita lost 7 pounds, but spent much of that time "starving and grumpy." She quickly gained the weight back after returning to her normal diet. It is this most recent "failure" that drove Sarita to seek the help of Opal, a health coach whom Sarita met at a health fair at her office building.
What would be an appropriate first step for Sarita's weight-management program, and how can Opal best introduce her to a more well-rounded approach to weight loss?
During their first session together, Opal explores Sarita's history with weight loss and notes that it is marked by large fluctuations as Sarita started and stopped fad diets over the years in a quest for a quick fix. They discuss the need for true lifestyle change, and Opal attempts to redirect Sarita's focus on appearance by explaining the many health benefits of very moderate weight loss. At 160 pounds, Sarita is surprised to learn that losing as little as 8 to 16 pounds and maintaining that weight loss would significantly improve her overall health. Sarita admits that her goal has always been to lose 40 pounds as quickly as possible.
Opal recognizes that Sarita is in the preparation stage of behavioral change, so she uses some very specific strategies to move her into the action stage. First, she helps Sarita identify obstacles and develop solutions to overcome them before they actually confront her. In addition, Opal finds that Sarita's self-efficacy for adhering to dietary changes appears to be fairly high, but that when it comes to exercise, her self-efficacy is extremely low.
